This Telehealth Informed Consent ("Consent") explains the nature of telehealth services provided by GLP ONE and the licensed healthcare providers who deliver care through our platform. Please read this document carefully before using our telehealth services.

By using our telehealth services, you acknowledge that you have read, understand, and agree to the terms outlined in this Consent.

What is Telehealth?

Telehealth involves the delivery of healthcare services using electronic communications, including:

  • Asynchronous Communication: Secure messaging between you and your healthcare provider, where responses are provided within a specified timeframe rather than in real-time
  • Synchronous Communication: Real-time video or phone consultations with your healthcare provider
  • Remote Monitoring: Collection and transmission of health data for provider review

Telehealth allows you to receive medical consultations, diagnoses, treatment recommendations, and prescriptions without an in-person visit to a healthcare facility.

Benefits of Telehealth

Telehealth services offer several potential benefits:

  • Convenient access to healthcare from your home or other private location
  • Reduced travel time and associated costs
  • Decreased exposure to contagious illnesses in waiting rooms

Risks and Limitations of Telehealth

Technology-Related Risks:

  • Technical failures may interrupt or terminate your consultation

  • Poor audio or video quality may affect the provider's ability to assess your condition

Clinical Limitations:

  • Your provider cannot perform a physical examination
  • Some conditions require in-person evaluation and cannot be adequately assessed via telehealth
  • There may be delays in receiving care due to technology failures or communication issues
  • Your provider may determine that telehealth is not appropriate for your condition and refer you to in-person care

Medication Considerations:

  • Some medications cannot be prescribed via telehealth due to regulatory restrictions
  • Without physical examination and laboratory testing, certain diagnoses may be presumptive
  • You are responsible for reporting all current medications to avoid potential drug interactions

Your Rights as a Patient

You have the right to:

  • Receive the same quality of care via telehealth as you would receive in person
  • Request a copy of your medical records
  • Refuse telehealth services at any time without affecting your right to future care
  • Ask questions about your diagnosis, treatment options, and the qualifications of your provider
  • Privacy and confidentiality of your health information in accordance with HIPAA
  • Know the identity and credentials of the healthcare provider treating you
  • Request an in-person referral if you prefer or if your condition requires it

Your Responsibilities as a Patient

To ensure safe and effective telehealth care, you agree to:

  • Provide accurate and complete information about your medical history, current symptoms, and medications
  • Notify your provider of any allergies or adverse reactions to medications
  • Inform your provider if you have a primary care physician or other healthcare providers involved in your care
  • Be in a private, safe location during your telehealth consultation
  • Follow your provider's instructions regarding treatment and follow-up care
  • Contact your provider or seek emergency care if your condition worsens
  • Not record telehealth sessions without the express consent of your provider

Privacy and Confidentiality

Your telehealth consultations are confidential and protected under the Health Insurance Portability and Accountability Act (HIPAA). We use secure, encrypted technology to protect your health information. However, no system is completely secure, and there is a small risk that your information could be intercepted.

Prescriptions and Pharmacy Services

If your healthcare provider determines that a prescription is medically appropriate:

  • Prescriptions will be transmitted electronically to our partner pharmacy or a pharmacy of your choice
  • You understand that compounded medications are prepared specifically for you by licensed compounding pharmacies and are not FDA-approved products
  • You are responsible for taking medications as directed and reporting any side effects to your provider
  • You will not share, sell, or give your medications to others
  • Controlled substances may have additional restrictions or may not be available via telehealth

Emergency Situations

Telehealth services are not appropriate for medical emergencies. If you experience any of the following, call 911 or go to your nearest emergency room immediately:

  • Chest pain or difficulty breathing
  • Signs of stroke (sudden numbness, confusion, trouble speaking or walking)
  • Severe allergic reactions
  • Uncontrolled bleeding
  • Thoughts of harming yourself or others
  • Any condition you believe requires immediate emergency attention

Costs and Payment

You are responsible for the costs associated with telehealth services as outlined in our pricing and Terms and Conditions. Costs may include:

  • Consultation fees
  • Medication costs
  • Shipping and handling fees
  • Follow-up appointment fees

GLP ONE does not bill insurance. You will be provided with a receipt that you may submit to your insurance company for potential reimbursement, though reimbursement is not guaranteed.

Consent to Telehealth Services

By checking the consent box during registration or by using our telehealth services, you acknowledge and agree that:

  1. You have read and understand this Telehealth Informed Consent
  2. You understand the benefits, risks, and limitations of telehealth
  3. You consent to receive healthcare services via telehealth
  4. You understand that telehealth is voluntary and you may withdraw consent at any time
  5. You agree to provide accurate health information to your provider
  6. You understand that a telehealth consultation does not guarantee a prescription
  7. You will seek emergency care when appropriate

Withdrawal of Consent

You may withdraw your consent to telehealth services at any time by:

  • Notifying your healthcare provider during a consultation

  • Updating your preferences in your account settings

Withdrawal of consent will not affect any services already provided or your right to seek future care.
