Warnings and Precautions

1. PancreatitisSemaglutide has been associated with pancreatitis, even at lower doses. Stop using this medication and contact your healthcare provider immediately if you experience severe abdominal pain that may spread to your back, with or without vomiting.

2. Diabetic Eye ComplicationsPatients with a history of diabetic retinopathy should be monitored during therapy, as rapid improvements in blood sugar control have been associated with temporary worsening of diabetic eye disease. Report any vision changes to your provider.

3. HypoglycemiaIf you take other diabetes medications, particularly insulin or sulfonylureas, microdose semaglutide may increase your risk of low blood sugar. Know the signs: dizziness, sweating, confusion, trembling, and rapid heartbeat.

4. Kidney EffectsGLP-1 medications have been associated with kidney problems, particularly in patients who experience significant dehydration from GI side effects. Stay well-hydrated and report severe or prolonged nausea, vomiting, or diarrhea.

5. Gallbladder ProblemsSemaglutide may increase the risk of gallstones and cholecystitis. Seek medical attention for sudden, severe pain in the upper right abdomen, especially if accompanied by fever or jaundice.

6. HypersensitivitySerious allergic reactions have occurred with semaglutide. Signs include facial swelling, severe rash, difficulty breathing, and anaphylaxis. Discontinue immediately and seek emergency care if these occur.

7. Mental HealthWhile uncommon, reports of mood changes and suicidal ideation have been associated with weight loss medications. Contact your provider immediately if you experience new or worsening depression, anxiety, or thoughts of self-harm.

Common Side Effects

At microdose levels, side effects are typically mild. You may experience:

  • Mild nausea (most common initially)
  • Fatigue
  • Headache
  • Mild gastrointestinal discomfort
  • Injection site reactions
  • Slight changes in appetite

These effects often resolve within the first few weeks of therapy.

Pregnancy and Breastfeeding

Pregnancy: Do not use microdose semaglutide during pregnancy. Animal studies indicate potential harm to the developing fetus. Stop this medication at least 2 months before planning to conceive.

Breastfeeding: Whether semaglutide passes into breast milk is unknown. Discuss with your provider whether to discontinue nursing or the medication.

Drug Interactions

  • Insulin and sulfonylureas (dose adjustments may be needed)

  • Levothyroxine
